Who Is Eligible for Hospice Care?
A patient is eligible for Hospice if he or she has a life expectancy of six months or less and chooses to elect Hospice rather than curative treatments. The patient's eligibility must be certified by their physician and the Hospice medical director.

- Pain and symptom management in your home or an extended care facility
- Personal care visits
- Continuous care, if needed, for symptom management
- Inpatient care for symptom control or respite stay to assist primary caregiver
- Spritual and emotional support for you and your family and friends
- Medical supplies and equipment and medication related to your hospice diagnosis
